On the recordings you can hear the phone ringing (very loudly) for all Incoming calls .  why does it do that and can it be fixed?

For incoming calls, please use following settings to achieve "start recording only after the call is answered":
- Call Settings - Incoming Calls - Auto Detect Answered State: Yes
- Call Settings - Incoming Calls - Auto Detect Max Time (Seconds): 3
- Call Settings - Incoming Calls - Restart Recording Upon Answered: Yes

For outgoing calls, because the recorder (any recorder in the market) can't detect if the call is answered or not, so all recorders can't achieve "start recording only after the call is answered". However if you root your phone Boldbeast Recorder can achieve it. After root please use following settings:
- Call Settings - Outgoing Calls - Auto Detect Answered State: Yes
- Call Settings - Outgoing Calls - Auto Detect Max Time (Seconds): 3
- Call Settings - Outgoing Calls - Restart Recording Upon Answered: Yes
